Applicants who provide a waiver, such as a scanned copy of a Social Security Disability Benefit notice (SSDI notice), a receipt of Supplemental Security Income Payment (SSI receipt), an Iowa Medicaid member card, or a Hawk-I member card, are eligible for a $25 price reduction.
